Another interesting case! Cyst or Synovial Sarcoma in this 10 year old child? The second image is with contrast! And the answer is SYNOVIAL SARCOMA! Synovial sarcomas can have a variety of radiological presentations and occur in all age groups. They can occur anywhere including within and around joints! This is very small and was read as a cyst. The contrast enhancement was concerning to me. It was carefully removed. Turned out to be a Synovial sarcoma. Returned to surgery for a wide resection. The knee joint capsule was reconstructed by rotating the sartorial muscle. The child is 1.5 years postop. No evidence of disease. The patella tracks normally and he has normal knee and extremity function.
